視床松果体部を占拠した小児側脳室髄膜腫

Ventricular Meningioma occupying the Thalamic and Pineal Region in an Eleven-Year-Old Child

Ⅰ．はじめに

　脳室内髄膜腫と呼ばれる腫瘍が実際に，側脳室の脈絡叢から発生するものなのか，あるいは脈絡組織（tela chorioidea）から発生して，二次的に脳室内に突出するものなのかにはいろいろ問題がある^29）．しかし脈絡組織から発生して松果体部に髄膜腫が発見された症例^12）も事実存在する点から考えると，これらは区別されるべきではないかと思われる．最近，われわれは中間脈絡三角（triangular velum interpositium）に発生し，側脳室内に突出したと思われる小児髄膜腫の1例を経験し，その占拠部位より松果体腫瘍に対すると同様な外科的アプローチを行ったので，この部の髄膜腫について文献的考察を行い，その手術経路について検討した．

A case of lateral ventricular meningioma was described. The patient, an 11-year-old boy have had a history of clumsy left hand since four-year-old and one generalized seizure at nine-year-old. Several focal seizures and one generalized seizure, and progressive clumsiness of the left extremities with a tremor of the left hand have been presented recently, despite treatment with anticonvulsant.

On admission, slight left hemiparesis and intentional tremor of the left hand was observed without any signs of increased intracranial pressure.
