松果体部腫瘍

PINEAL REGION TUMOR……WITR SPECIAL REFERENCE TO SURGICAL TOTAL REMOVAL OF THE TUMOR

緒言

　松果体は古くGalenの時代より知られているが，松果体疾患として最も問題となる松果体腫に関してはKrabbe （1923）がpinealomaなる名称のもとに報告して以来，多数の報告例がある。しかしながら，その治療は非常に困難であつて，剔出手術は脳外科手術中最も困難なものの1つとされており，これは松果体の位置が脳の深部にあり，かつ重要な血管が囲繞し，自律神経中枢その他重要な中枢がその周囲に密接しているからである。しかるに最近の麻酔，術前術後管理の進歩は脳外科手術手技の進歩と相俟つてこの部の腫瘍の剔出手術においても新分野をひらき，手術成功例が次第にみられるようになつたことはまことに喜ばしいことである^1）。

　吾々は岡山大学第1外科教室において松果体部腫瘍15例を経験しているが，その内訳はpinealo-ma 9例，pinealoblastoma 1例，pinealoterato-ma 1例，teratoma 2例，astrocytoma 2例である。これらのうち全剔出を行つたもの4例，被膜内容全剔出を行つたもの1例，部分剔除を行つたもの4例，部分剔除とTorkildsen手術を合併して行つたもの1例，Torkildsen手術のみを行つたもの2例となつているが，残りの3例は全身状態が悪く脳室穿刺，あるいは脳室ドレナージのみしか行いえなかつた。

Fifteen cases of pineal region tumor were reviewed. They were histologically: 9 cases of pinealoma, 2 cases of teratoma, 2 cases of astrocytoma and each one case of pinealo-teratoma and pinealoblastoma. Among these cases, surgical total removal was per-formed on 5 cases, in which one case of pinea-loma died on the fourth postoperative day, one of pinealoma after four months, one of teratoma after 16 months, while two cases of pinealoma and pinealoteratoma, are actively living one year after surgery.

Dandy's transcallosal technique was used for surgical removal under hibernations an-esthesia, which was considered most adequate to the operation of this region with regard to the influences upon the hypothalamus and brain stem.
