開頭術後に発症した全身性カンジダ症の2例

Ⅰ．はじめに

　外科的合併症のうち，術後感染症は各種の予防法や治療法の発達した現在でもなお重要な課題の一つである．脳神経外科領域においても，約3-5％の術後感染症が報告されており，感染症の成立に及ぼす各種因子について詳細な検討がなされている^{2,15,19,20）}．　しかし，これらの報告にみられる感染症は，ほとんどが一般細菌に起因した感染症であり，開頭術後に発生した真菌感染症は外傷例の報告に散見するにすぎない^7）．今回，私達は開頭術後に全身性カンジダ症が発症し，死に至った2症例をあいついで経験した．この2症例の経験を報告するとともに，本症も含め，脳神経外科領域においても今後増加が予想されるopportunistic infectionについて言及し，あわせて本症の予防と治療について若干の文献的考察を加えたい．

Two cases of postoperative systemic candidiasis in neurosurgical practice are reported.

Postoperatively these patients showed signs and symptoms of systemic infection and unfortunately died in spite of intensive chemotherapy. In both cases systemic candidiasis became evident by histological examinations at autopsy. Long use of antibiotics and aggressive nosocomy were supposed to be the cause of the systemic candidiasis.
