Gelfoam powderを用いた髄膜腫に対する術前塞栓術にて腫瘍出血を生じた1例—その出血機序についての検討

I．はじめに

　術中の出血量を減少させる目的のために，髄膜腫に対して術前栄養血管塞栓術が，すでに多くの施設にて行われており，その有効性も広く認められている．

　しかし，それによる合併症も少なからず報告されているが，塞栓物質の頭蓋内血管への流入によるものを除いては，いずれも比較的軽微なものが多く，塞栓術によると思われる腫瘍周囲への血腫形成の報告は，われわれが渉猟し得た範囲では，わずか数例^10,12,14）にすぎない．

Preoperative embolization for intracranial menin-gioma has been performed at many institutions and its effectiveness has been well recognized. The complications of embolization such as facial pain, fever and facial nerve palsy, et al, were mild and temporary except embolus migration into intracranial vessels, but a peritumoral hemorrhage clue to pre-operative embolization was extremely rare. Recently we have experienced such an unusual complication, then we describe this complication here and discuss the mechanism of hemorrhage briefly.
