放射線壊死の病理学的研究—剖検例に基づく脳腫瘍の完全壊死像の解析

Pathological Investigation of Radiation Necrosis: Complete Necrosis of Brain Tumors Observed in Autopsy Cases

はじめに

　すでに1920年代から，脳腫瘍なかでもgliomaに対する放射線治療は行なわれてきた^31）．それに伴う腫瘍および脳組織のいわゆるradiation necrosisの剖検例に基づく報告はO'Connell（1937）4例^29），Wachowski（1945）2例^37），Pennybecker ＆ Russell（1948）2例^32），Zeman（1949）4例^38），Eicke（1952）1例^10），Lampert（1964）1例^22）などがある．また，頭部皮膚疾患や顔面頭蓋の悪性腫瘍あるいは下乖体疾患などに対する放射線治療に伴う「脳組織」のradiation necrosisについても，諸外国では古くから報告がある^{4，6，8，9，11，19，22，24，25，27，32）}．

　他方，放射線治療は本邦でも脳腫瘍の非手術的療法の一環として広く行なわれてきているにも拘らず，頭蓋内組織におけるradiation necrosisの報告は少なく^{15，34-36）}，とくにgliomaの剖検例に基づく報告を見出し得ない．

The paper is a morphological description of six cases of severe necrosis of the tumors undoubtedly attributable to the irradiation, found in the course of studying the effect of radiation therapy in 84 autopsy cases of intracranial tumors including 59 gliomas.

1) In two cases of craniopharyngioma and of pineal teratoblastoma (biopsied previously), massive, hyalinous, scar-like connective tissues were present without residual tumor cells.
