Ⅰ．はじめに

　急性期脳梗塞は急性心筋梗塞と同じく循環器救急疾患であり，速やかな診断と治療開始が重要である¹⁷⁾．2005年10月に虚血性脳卒中に対する超急性期の治療としてrt-PA（recombinant tissue plasminogen activator）静注療法が承認され，全国各地に脳卒中急性期治療施設が確立してきている．rt-PA静注療法は一定の治療基準や方法が確立されているが，適応外や他の虚血性脳卒中では，必ずしも統一した治療がなされていない．脳神経外科医は，救命あるいは手術を目的として原因となる血管病変に挑戦してきた一方で，高齢の軽症脳梗塞患者への初期診療にあたる医師も少なくない．ここでは急性期脳梗塞の内科治療について，われわれが強調してきた初期対応，画像陰性時の臨床判断²¹⁾と治療開始決断¹⁹⁾，ガイドラインに基づくスタンダードな治療^7），急性期から始まる統合的な治療などについて解説してみたい．
