An Autopsy Case of Infantile Subacute Necrotizing Encephalomyelopathy

I．はじめに

　乳幼児期あるいは少年期に，傾眠，視聴力障害，四肢の痙直などの特異な臨床像をしめし，主として脳幹部における特異的な組織変化と病巣分布を伴う，比較的稀な疾患，すなわちinfantile subacute necrotizing encephalomyelopathyを初めて独立疾患として記載したのは，1951年subacute necrotizing encephalomyelopathyの名のもとに報告したLeigh^13）であるとされており，後にRichter^18）によりinfantileの語がこれに付加された。

　ところが，すでに1934年，T. Tanaka^20）がso called breast milk intoxicationとして本邦から報告した症例と，一部の疑義もあるが，本症は同一疾患であろうと考えうることがFeigin ＆ Wolf^8），およびRichter^18）などによつて指摘されていることは興味深い。

　A 12 year old boy expired under the suspicion of brain stem disorder characterized by suppression of respiratory movement during the sleep.

　He has been suffered from the insidious but progressive neurological disturbances such as unsteadiness of gait, ophthalmoplegia, dysphagia, dysarthria and impediment in vision since infancy. He had several fits of characteristic riespiratory suppression in the course of his illness. He was born of consaguineous parents and his sibling also died suddenly of unknown course at the age of 4.
