Pyruvate dehydrogenase complex (PDHC) is an important enzyme that suplies a substrate to the TCA cycle for producing energy. The defi-ciency of PDHC causes a biochemically functional abnormality of mitochondria, which have been demonstrated in a number of different syndromes, ranging from fatal congenital lactic acidosis and subacute necrotizing encephalomyelopathy (Leigh's encephalomyelopathy) to various intermittent move-ment disorders and ataxias which may begin somewhat later in life.Signs of central nervous system dysfunction dominate the clinical picture in this disorders, because brain virtually depends on glucose oxidation for their energy requirments.
