覚醒剤精神病と精神分裂病

Methamphetamine psychosis and schizophrenia: Clinical and biomedical correlations.

はじめに

　覚醒剤の乱用が嗜癖（依存）を生じやすいとする報告は1930年代からみられるが，精神病様状態を呈するとする最初の報告は1938年覚醒剤によるナルコレプシー患者の治療中に生じた妄想反応に関するもので，YoungとScoville^1）によってなされた。その後のアメリカおよびヨーロッパにおける覚醒剤精神病の報告は症例数が必ずしも多くはない。

　欧米で用いられた薬品名はamphetamine（以下AMP）であったが，主として経口的摂取によるもので，それだけに精神病症状の発呈例は少なかったものと思われる。ヨーロッパを中心とする妄想性精神病像の発症例の臨床的検討が，Connellによってまとめられ，その書はアンフェタミン精神病の古典的文献として有名である^2）。わが国の場合は，第二次大戦後に軍関係で使用されていた大量のmethamphetamine（以下MAP）の在庫品が市場へ放出されたことが，乱用の契機となり，最初は，作家や芸能人が用い出し，それが労働者へ及んだ。

Abstract

　Methamphetamine (MAP) abuse was seen on a large scale in Japan during the post-war period (the first abuse period). The number of abusers extremely decreased by strict regal control in 1958. But the abusers have been gradually increasing again since 1970 (the second abuse period) and MAP psychosis caused by chronic abuse of the drug also is on the increase.

　MAP psychosis shows the symptoms varing from transient hallucinatory delusional states which are of brief duration, to a more prolonged states closely mimicking chronic schizophrenia.
