薬物感受性のサーカディアンリズム—組織感受性リズム仮説

A theory of circadian susceptibility rhythm of tissues to drugs

はじめに

　薬物の効果が投与された時刻によって異なるという事実が明らかにされたのは，ごく最近のことである。従来の医学は"時刻"の有する重大性について一般に無関心であり，生体がリズムをもって常に変動していることを理解することなく，不動のものとしてとらえる傾向が強かったようである。薬理学の実験においても，やはり"時刻"は，ほとんど注意を払われることがなかった。臨床における投薬についてもまたしかりである。

　最近，この分野の研究が進み，多くの事実が明らかにされてきた，薬物効果のリズム現象は，ごく少数の限られた薬物，または特定の作用に限られたものではなく，ごく一般的な現象であることが明らかになり，さらに発現機序についても各種のアプローチを通じて解明が進んでいる。著者らは，1975年以来，従来の薬物代謝リズム原因説に対し，組織感受性リズム原因説を提唱してきたが，最近，これを支持する多くの事実が報告されつつある。本稿では，薬物感受性リズムについて，これまでに分っている法則を述べたあと，その発現メカニズムについて考察したい。

Abstract

Circadian rhythm of pharmacological effect has been reported on more than 80 drugs. Now, it may be considered a phenomenon which can be seen on any drugs. The rhythm is considered as "endogenous", because it shows "free run" under constant conditions.

There are many studies on the mechanisms of the rhythm of pharmacological effects. In such researches some authors found the rhythms of absorption, excretion, or metabolism of drugs. Such studies are interesting, but less important because the rhythms may compensate each other in a living organism.
