Neuroblastic Differentiationを示した松果体部腫瘍の1剖検例

I．はじめに

　松果体部に発生する腫瘍にはtwo cell patternを示す松果体腫pinealoma（atypical teratoma）のほか，gliomaや低頻度ではあるが松果体実質細胞に由来すると考えられているpineocytoma,pineoblastomaなどが知られている^16）。これらの腫瘍中，最も頻度の高いpinealomaについてはその組織由来に関して，現在では松果体実質に由来するものではなく，生殖腺に発生するseminoma,dysgerminomaと類似のgerm cell tumorと考える学者が多い^1,11,16）。これに対しpineocytoma，pineoblastomaの報告例は少なく，とくにpineocytomaの超微像に関しては1例の報告^12）があるにすぎない。

　今回，われわれは松果体部に発生した腫瘍の剖検例を経験し，その組織学的，電顕的検索を行なったので報告する。腫瘍はpineocytomaの組織学的特徴を示してはいるが，神経細胞系への分化が明らかで，特異な病理形態を示しており，その組織由来に関して若干の考察を加えた。

Abstract

A case of pineal region tumor is presented in a 36 year-old woman. She was admitted to the hospital on February 3, 1976, with chief com-plaints of headache, reduced vision and generalized fatiguability for three months' duration. Physical examination showed bilateral chocked discs with loss of visual acuity. There was no neurological dificits. Ventriculogram showed a dilatation of the ventricular system with a filling defect in the posterior wall of the third ventricle. EMI scan disclosed a high density area around the third ventricle.
