前額断面を加えたコンピューター断層撮影の臨床的意義

I．緒言

　CT（computed tomography）が本邦でも総数200台に及ぶに至りほとんどの頭蓋内疾患の検討がなされる現状だがその大半はhorizontal（transverse）sectionである。このことは頭部X線検査の弱点を補う断層面での所見が得られることで当初驚異的であり，立体的把握が可能になることで満足すべきものであった。それゆえにいわゆる"cranial CT scanner"はhorizontal sectionを得る目的でのみ作られている。しかしCTに関する技術が日進月歩の発達をとげるに呼応してCT所見が従来の他検査に対する補助的役割から疾患によっては主体的役割を占めるようになるにつれて，さらに多くのinformationを得ようとする傾向が著明となって来た。このような傾向の一つに他の断層面，すなわちfrontal（coronal）section,sagittal section,oblique sectionなどがあり疾患によってはきわめて臨床的価値の高いものがある。本稿ではこのなかでfrontal sectionを中心に述べる。

Abstract

　The computed tomography (CT) would be routine examination in neurosurgical clinic. The information of CT is going to be demanded more and more for precise diagnosis without any invasive examination. The additional frontal section has several advantages in making differential diagnosis and getting useful preoperative informations in several cases. The first, it is useful for differential diagnosis and making diagnosis of small lesions around the sella. The second, in several giant aneurysmal cases without operative indication, any angiography would be no longer necessary after enhanced CT scan.
