悪性脊髄腫瘍の手術的治療

Surgical treatment of spinal cord tumors

はじめに

　脊髄腫瘍の発生率は，人口10万人に対し1〜2人といわれ，脳腫瘍の約1/7〜1/10にすぎず比較的少ない腫瘍といえる。その組織学的種類の内容は諸家によると表1のごとくである。

　しかしこれは癌の2次的転移あるいは周囲組織の癌からの浸潤を除いた数字で，もしこれらを含めると大分様相が変わってくる。私がOxfordのクリニークで集計した表（表2）でも¹），脳外科の手術の対象になった転移癌だけでも全脊髄腫瘍の25％を占めており，手術の対象にならない症例を含めれば，この数字はもっともっと増加するであろう。

　Tumors of the spinal cord are classified into 3 groups according to topographical criteria; epidural (extradural) tumors; intradural and extramedurally tumors; and intramedurally tumors.

　Most of the extradural tumors are histologically malignant, including multiple myeloma (plasmacytoma), sarcomas, malignant lymphomas (lymphgranulomatosis) and metastatic carcinoma. With recent advances of radiotherapy and chemotherapy, surgical results for myeloma and lymphgranulomatosis have improved markedly and the prognosis of these diseases are no longer hopeless.
