脳腫瘍の血管撮影

Angiographical diagnosis of brain tumors

I．はじめに

　脳腫瘍の診断にコンピューター断層撮影（CT）が採用されてから血管撮影の役割も大きく変わった¹）。従来最も大きな課題であった腫瘍の有無の診断，すなわち存存診断はほとんどCTの役割となり，血管造影は腫瘍の拡がりや二次的な変化など特殊な問題の解決に焦点を合わせるようになった。腫瘍の組織的性格の判定はCT，RI検査，血管造影などの協調によっても難しい場合もあるが，半数以上の例においてそれが可能になってきたのが最近の状況である。この綜説では血管造影から得られる脳腫瘍の所見のうち腫瘍に出入する血管系，腫瘍陰影，腫瘍周囲の血管像などについて述べることにする。そのほとんどは多くの成書に記載されているところである。

Abstract

Since the computer tomography (CT) debuted as an ideal examination of a brain tumor, cerebral angiography should play a role different from the previous one. To prove presence or abscence of an intracranial mass is not a major part for the angiography. Demonstration of some details of the intracranial state induced by a brain tumor, including exploration of factors of tumor pathology, is now subject of the angiography.

A brain tumor, if it requires blood supply much more than normal tissues, has proper vessels within its structure.
