緒言

　1961年Birks ＆ Maclntosh^1）がAcetylcholine（以下Achと略）による化学的Synapse伝導を解明して以来，神経筋伝導に関与するAchの合成，貯蔵，Mobilization Releaseの問題が検討され，Myastheniaの本態追求が行なわれて来た。ここに提示する症例はClassical型のMyasthenia gravisとして抗Cholinesterase剤で治療中，症状が悪化した患者で，この増悪期間中行なつた臨床，電気生理，薬理学的分析の結果，部分的にはEaton-Lambert型に類似していて，普通のMyastheniagravisとは異なる神経筋伝導blockの状態にあることが示唆され，Calcium剤治療を試みて奏効をみたものである。

Since an attractive suggestion of Birks and Mac-Intosh who investigated the synaptic transmission by acetylcholine (Ach), the synthesis, storage, mobilization and release of Ach of the nerve terminal have been considered to range in series and operate upon each other. In line with this concept, much attentions have been paid to the problems regarding neuromuscular transmission in myasthenia.
