Locked-in症候群の定義とその背景について

The "locked-in" syndrome

　1966年に出版されたPlum ＆ Posnerの名著"Diagnosis of Stupor and Coma"に，外見は無動，無言であるが，意識や精神機能は正常で，随意的な眼球の上下運動のみが保たれ，しかもそれを介して意志の疎通が可能な特異な患者が記載され，locked-in症候群と名づけられている（Plumら，1966³⁷）。これは見かけはakinetic mutismと似た状態を呈しているが，本質的には明確に区別されるべきもので，その認識は臨床上きわめて重要である。しかし，その原著が本であったためか，locked-in症候群はあまり注目されずに最近に至っている。

　locked-in症候群として報告された症例は外国では19例に及び，本邦では著者らが1972年に第一例を報告してから（沢田ら，1973^41）），すでに9例に達している。ここではすでに発表された文献を中心に，自験例をも加えてlocked-in症候群の概念に触れ，定義とその背景づけを行なってみたい。

　The "locked-in" syndrome in a term first coined by Plum and Posner in 1966 to describe the tetraplegic, mute but fully alert state. Since then, there have been several case reports and further descriptions of the clinical findings. Althouth the "locked-in" syndrome somewhat resembling akinetic mutism, these two states should be differentiated. The pathogenesis and particularly the accompanying suffering are entire different.
