脳原発性悪性リンパ腫の研究—螢光抗体法による検討

Demonstration of intracytoplasmic immunoglobulin (s) in a primary malignant lymphoma of the brain

Ⅰ．序

　悪性リソパ腫はリンパ網内系を発生母地とする腫瘍の総称であるが，リンパ網内系を欠く臓器や組織にもしばしば原発し，extranodal lymphomaとして知られている。脳原発性悪性リンパ腫はこのextranodal lymphomaの代表例であり，その研究の歴史は長く，すでに多くの優れた報告や総説がある^1）。英文論文だけでも数十篇以上，記載された症例は百数十例以上になる^1）。本邦例の報告も少なくない^1,2）。脳原発性悪性リンパ腫の分類および呼称については長年にわたり議論が続けられて来ているが，悪性リンパ腫を仮にrcticulum cell sarcoma，lymphosarcoma，Hodgkin病に大別すると，報告例の大多数はreticulum cell sarcomaあるいはその近似腫瘍の範晴に入り^1,2），その発生母細胞としては髄膜や血管周囲性の組織球，血管外膜の未分化細網細胞，脳実質中のミクログリアなどが考えられている^3）。しかし真の発生母細胞は現在なお未定であり，それに伴う多くの問題が未解決のまま残されている。

Immunofluorescent study, using formalin-fixed, paraffin-embedded tissues and highly specific anti-bodies, revealed that immunoglobulin chains (γ,κ and λ) were simultaneously present in single tumor cells of a primary malignant lymphoma of the brain. Such cells were estimated to be 10 to 30% of the total cell population. Cytologic analysis of the individual immunoglobulin-positive and negative cells by means of additional hematoxylin-eosin stain to the same section suggested that these immunoglobulin chains were not of exogenous origin but were synthesized at the site and that the tumor, therefore, belonged to B cell system.
