Metabolic fuels for exercising muscles and inborn errors of the fuel utilization in skeletal muscles of man

Ⅰ．骨格筋におけるエネルギーの生成系

　骨格筋における運動エネルギーは，解糖および呼吸によって獲得される。これらエネルギーの産生系の主要な基質は，いうまでもなく，糖質（グリコーゲン，ブドウ糖），長鎖脂肪酸およびアセトン体（β-ハイドロキシ酪酸，アセト酢酸）である（図1）。アミノ酸も脱アミノを受けてエネルギー生成系へ流入し得るが，エネルギー産生に占める役割は小さいと考えられている。筋肉は摂食後を除いて一般にアミノ酸を放出する器官であり，運動時にはとくにアラニンの放出が増す^1）。この場合，筋運動に伴う解糖の促進により生成の亢進した焦性ブドウ酸がアミノ基転移を受けてアラニンに転化したものが大部分を占めている。アラニンは筋蛋白の7〜10％を占めるに過ぎないが，放出されるアミノ酸の30％以上はアラニンである。

　さて，上記の主要な諸基質からエネルギーを生成する系について，臨床との関連を考慮しながら大切な諸点を要約しておきたい。

　Carbohydrates (glycogen, glucose), long-chain fatty acids and acetone bodies (β-hydroxybutyrate, acetoacetate) can be utilized efficiently in exercising skeletal muscles. However, the degree of participation of these substances in energy production depends on nutritional conditions, duration of work-performance, and intensity of muscular exercise.

　Several clinical entities indicating inborn errors of metabolic pathways for these substances have been discovered recently, and the clinical observations of such inborn errors of metabolism could throw further light on the role of each substance for muscular exercise in man.
