いわゆるOcular Myopathyの筋病理所見—とくにMitochondriaの異常について

Pathological findings of skeletal muscle in ocular myopathy: Especially abnormalities of mitochondria

　進行性外眼筋麻痺は1868年von Grafe^1）により最初に報告され，1951年Kiloh^2）らの外眼筋の組織学的検討により従来の神経原性の考えからocular myopathyと考えられている。最近，ocular myopathyの臨床像を示す症例に網膜色素変性，心筋障害，髄液蛋白増加，脳波異常などを伴い，骨格筋の生検で著明なミトコンドリア異常^3）を認めることが報告されている。私どもは臨床的にocular myopathyと考えられる10例に筋生検を行ない，1例には肝生検をも行ない，骨格筋のみならず，肝細胞のミトコンドリアにも異常を認めたので報告する。

　10 cases of the so-called ocular myopathy with abnormal muscle mitochondria were reported.

　All 10 cases presented with external ophthalmoplegia and ptosis, in addition, pigmentary degeneration of retina, sensory deafness, brachydachty, abnormalities of EEG and ECG, and elevated cerebrospinal fluid protein level were noted in some cases.

　Histological study on the skeletal muscle biopsies revealed "ragged-red fiber" in all cases. With electron microscopy, the major pathological changes of these muscle cells were confirmed to the abnormal mitochondria with paracrystalline inclusions.
