著明な組織球の出現と白質壊死を特徴とした脳炎の1例

An autopsy case of lymphohistiocytic encephalitis with multiple parenchymal destruction (Histiocytic medullary reticulosis)

I．はじめに

　われわれは，全身の細網症に伴い，中枢神経系の広汎な領域に著明な組織球の浸潤と，リンパ球の反応を認め，それに伴い，特異な白質の散在性の壊死，脱髄病変をみた例を経験したので報告する。

　脳病変は，細網細胞の出現に加え，著明なリンパ球の反応があって二次性脳炎とみなされるものであったが，基本的には，全身性細網症の脳病変として理解できる。

Clinically, a 23-year-old woman was admitted to the Hospital with intermittent fever and pul-monary infiltration. Wasting, hepatosplenomegaly, skin rash, purpura, uveitis, iriditis, jaundice, anaemia, and lymph node enlargement were the characteristic symptoms in the present case. Gen-eralized convulsion, hemiplegia and dementia were also noted. The duration of the illness was one year and 6 months.
