白血病の薬物治療中（L-asparaginaseその他）に生じた急性脳症とその脳病変について（付：病巣内に発見された核内封入体）

Ⅰ．まえがき

　小児急性骨髄性白血病（AML）経過中に生じた従来にない性質と激しさを示す脳病変を記載し，これと治療薬物，とくにL-asparaginaseとの関連を示唆したのは，著者らの1人本多らである（1971年）。これを含めここには5例の類似剖検例が報告される。この偶発脳症の原因や発生機序は私どもの検索によっても確定できなかった。また1例の脳病巣からはウイルス様核内封入体が発見された。こうした不幸な症例の検索において，そこにおこりえたであろう治療薬物の毒性，薬物による個体の防衛機構障害の問題をも考慮しつつ慎重に記載にとどめ，将来への一助とすることは，医療に関わる者の義務と思う。さらにこの循環障害性色彩の濃い脳病変が私たちの関心を引いたのは，これと肝脳疾患類瘢痕脳型の脳病変との類似である。一方L-asparaginaseの副作用のなかでも，肝および脳症状が目立つ（Clarkson et al. 1970）。ここには肝を中心とした全身代謝の障害の脳への影響，その形態的表現について，重要な示唆があると思われる。

An acute encephalopathy in 5 cases of AML (4 infants and 1 adult), treated with L-asparaginase and other various antileukemic drugs, was reported. The brain lesions were of the nature of diffuse circulatory disturbances or hypoxydosis and showed acute anoxic and edematous changes inthe acute cases and severe softenings of the cerebral hemispheres in the cases with prolonged course. Astrocytic nuclei were somewhat enlarged and vacuolated and fine carmin-positive granules were found in some of them and also free in the tissue.
