重症筋無力症の内分泌学的側面

Endocrinological Aspects in Myasthenia Gravis: with Special Emphasis upon Synthetic ACTH Therapy

　重症筋無力症はかつては神経―筋接合部の限局性障害として理解され，たしかに抗コリン・エステラーゼ剤によつて対症的には易疲労性などの臨床症状は著しく改善される。しかし本症と胸腺の関係^1,2），さらには自己免疫機序の関与などが注目されるに及んで，本症は全身性の疾患であり，ことに個体の複雑な内部環境の変動が，本症の寛解，増悪をくりかえす経過に微妙な影響を与えているものと想定されるようになつた^3）。したがつて本症に対する内分泌腺の影響はホルモンの神経―筋接合部への直接的な作用のほかに，ホルモンの生体の反応性への影響や，さらにはホルモンの既知の作用機序以外の可能性などをも再検討する必要がある。

　このような観点からわれわれは内分泌環境の本症の経過に及ぼす影響を調査し，さらに最近注目されているACTH大量療法^6,7,8）の意義を合成ACTHを用いて検討した。

Myasthenia gravis is not a disorder localized in the neuromuscular junction but a generalized and multisystemic disease. The changeable course of this disorder is to be elucidated by exploring the role of internal environment in myasthenia gravis and patient's reactivities to external stimuli.

(1) Among 30 cases with myasthenia gravis five (17%) were complicated with hyperthyroidism during the course of the disease. In four cases the appearance of hyperthyroidism was preceded by myasthenic syndrome.
