先天代謝異常症に対する遺伝子治療

Gene therapy for inborn error of metabolism

多くの先天代謝異常症は中枢神経症状を呈する。現在，骨髄移植がリソゾーム蓄積症やペルオキシゾーム病の中枢神経症状を加療または予防する目的で行われているが，ドナーが得られないことや，GVHDなどの問題点がある。それにかわる療法として遺伝子治療が多くの研究者によりモデル動物を用いて研究されている。多くのアプローチが検討されており，現在，有望なのはウイルスベクターを用いて直接脳に遺伝子を導入するアプローチや造血細胞や神経幹細胞を標的としてex vivoの遺伝子治療を行う方法などである。

CNS is one of the main affected organ in many inborn error of metabolism. So far, bone marrow transplantation is performed to treat CNS involvement of lysosomal storage diseases and peroxisomal disorders. However, bone marrow transplantation has several limitations, such as unavailability of donors and GVHD. As an alternative treatment, many investigators are currently trying to develop gene therapy using animal models. The many approaches are now being tested and these include direct gene transfer to brain using viral vector and ex vivo gene therapy using hematopoieitc cells and neural stem cells.
