重症脳損傷患者に対する脳低温療法と脳内ドーパミン補充療法

The brain hypothermia and cerebral dopamine replacement therapy for severe brain injury

はじめに

　これまで，脳損傷に伴う脳の病態は，脳循環障害と脳浮腫に伴う組織病態を中心に解析が行われてきた。これらの病態がその中に含むミクロの神経細胞がどの様なプロセスで死に至るかが，遺伝子障害や修復機構の研究によってようやくapoptosisやmaturationalneuronal deathの概略が理解されるようになってきた^18）。しかし，その具体的な治療となると，壊れた脳は治せないのでそこから発生する脳浮腫や脳循環障害を防止する脳組織損傷を中心とする二次的病態に対する脳保護治療が行われている^18）。

　一方，低体温療法は，脳虚血に神経細胞が耐えしのんで生き残るための手法として1954～1980年頃まで盛んに行われた時代がある，しかし低体温に伴う生体侵襲の病態の解明がなされなかったこともあって合併症が多く危険な治療としてその後行われなくなった。

We have presented a new concept of brain hypoxia oriented brain hypothermia treatments. All severe brain injury patients (148 cases) wereGCS<6. The masking brain hypoxia by brain thermo pooling, catecholamine surge induced cardiac dysfunction and intestinal vasodilatation, reduction of Hb-2, 3 diphosphoglyserate and lower oxygen delivery were major target of initial treatment. These specific brain hypoxia was only controlled by moderate brain hypothermia (34-32°C), oxygendelivery>800ml/min. and>100%.
