Treatment of demyelinating neuropathies: intravenous immunoglobulin (IVIg) therapy

　脱髄性ニューロパチーは，治療可能な病態が多く，またしばしば他の疾患と誤診されやすい。その正確な診断は治療への第一歩であり，電気生理学的な検査は必要不可欠である。大量ガンマグロブリン（IVIg）療法は，その作用機序は不明の点が多いが，血液神経関門の破壊を修復することが考えられる。本法は次第により多くの疾患で有効性が見出されつつある。比較的副作用は少ないといえるが，脳梗塞には十分注意を払う必要がある。

Demyelinating neuropathies comprise acquired and hereditary disease. Acquired neuropathies are mostly treatable, and hence its clinical recognition is crucially important. They are often mistaken as cervical and lumber spondylosis, monomelic neuropathies, or motor neuron diseases. Electrodiagnosis is the key for differential diagnosis. Chronic inflammatory demyelinating neuropathy and multifocal motor neuropathy readily respond to intravenous immunoglobulin, and recent clinical trial on Guillain-Barre syndrome revealed its efficacy and safety over plasma exchange.
