血管炎性ニューロパチーの発現機序

Pathogenesis of vasculitic neuropathy

　血管炎性ニューロパチーは全身性血管炎の一部として，中・小型血管炎症候群に多く見られ，時には末梢神経限局性の血管炎の形で見られる。基礎疾患によりその病態は異なるが，臨床的には多発性単ニューロパチーを呈し，病理像は多巣性の分布を示す軸索変性が主体で実験的虚血性ニューロパチーの変化と類似している。血管炎の発現機序についてまだすべてが明らかにされたわけではないが抗好中球細胞質抗体の発見や免疫学的研究の進歩により血管炎研究の分野に新たな展開が見られている。血管炎症候群，血管炎性ニューロパチーの発現機序を明らかにしていくことは速やかな診断と病態に即したより選択的な治療へと導く。新しい免疫学的知見をふまえた血管炎性ニューロパチーについての研究が必要である。

Vasculitis comprises a wide variety of syndromes characterized by inflammation and necrosis in the blood vessels, resulting in narrowing or occlusion of the lumen. Vasculitic neuropathy is often associated with vasculitic syndrome affecting middle or small sized blood vessels. Also, vasculitis restricted to peripheral nerves, that is, nonsystemic vasculitic neuropathy, has become accepted as a specific entity of vasculitis. Since peripheral nerves have a rich anastomotic blood supply, vasculitic neuropathy becomes evident only when multiple vessels are involved.
