Mutation of the a -subunit of muscle specific voltage-gated Na-channel causes either paramyotonia or hyperkalemic periodic paralysis. The slight increase in the extracellular K after meal or exercise leads the mutated channel in a state of non-inactivating mode, thus eliciting the entry of Na into the cell. This, in turn, causes depolarization and opening of normal Na-channel and caused further entry of Na into the cell and hence depolarization. This sort of vicious circle induces depolarization which underlies paramyotonia and periodic paralysis.
