Serotonin basic neuroscience discoveries are envolving at a very fast pace and are leading to innovation in clinical psychiatry and in drug development. This review provides an appraisal of the contribution of possible alterations in the function of serotonin receptor subtypes to the pathophysiology and treatment of various neuropsychiatric disorders, including schizophrenia, affective disorders, anxiety, panic disorder, obsessive-compulsive disorder, alcoholism and Alzheimer-type dementia. 5-HT-2 receptor has been postulated to play an important role in the pathogenesis of affective disorders based on the several findings including up-regulation of 5-HT-2 (receptor binding sites in the postmortem brain of suicide and depressed subjects, and hyperresponsiveness of 5-HT-stimulated Ca mobilization mediated by 5-HT-2 receptors in platelets from depressed patients. In this review it is also discussed whether or not glucocorticoid receptor is involved in the up-regulation of 5-HT-2 receptors, since a variety of neuroendocrinological abnormalities have been detected in depressive disorder including the increase in cortisol secretion and the attenuation of cortisol suppression to dexamethasone.
