 Various methods of treatment have been used in patients with HAM/TSP since the concept was proposed. Most involve modulation of the immune system and include corticosteroids, plasmapheresis, interferon-alpha, and high-dose gamma-globulin.

 Oral prednisolone was the first agent shown to be effective in HAM/TSP and is now the most common treatment; 74.0% of patients respond. Unfortunately, the effect is decreased with dosage reduction, and prolonged steroid treatment is often associated with serious complications. The mechanism by which corticosteroids modify HAM/TSP is still unclear. It is reported, however, that spontaneous proliferation of peripheral blood lymphocyte (PBL) seen in HAM/TSP patients is reduced by prednisolone treatment.
