右大脳半球梗塞後に生ずる左半側空間無視における病巣の研究

Study of lesions in left unilateral spatial neglect after right cerebral infarction.

I．はじめに

　半側空間無視（unilateral spatial neglect，以下USNと略）とは，脳損傷部位と反対側の外空間にある対象を無視する症候であるが，臨床的には右半球損傷による左半側空間無視が非常に多い。

　今世紀に入り，Brain^4），PatersonとZangwill^21）がこの症候を詳細に報告しているが，これらの報告以後，その責任病巣については，左半側空間無視と右頭頂葉病巣との関連が重視されるようになった。その後，右大脳半球の頭頂葉^7,10）のみならず前頭葉^9），後頭葉^15），視床^27），内包^5），島葉^3），基底核^11）の各領域の病巣による左半側空間無視例が報告されているが，これらの研究報告の間では「半側空間無視」の診断基準ないしは規定が明確に統一されているとはいえない。

The anatomical correlates of unilateral spatial neglect (USN) was investigated.

Ninety six right-handed patients with the right cerebral infarcts, provided that patients with de-mentia or cortical atrophy had been excluded, were consecutively examined for USN on the following tests: line bisection, line cancellation, figure copying, clock drawing, and pictures pointing.
