 This article reviews the host factors involved in HTLV-I-asociated myelopathy/tropical spastic paraparesis (HAM/TSP) in comparison with those in adult T-cell leukemia (ATL). The geographical distribution of HAM/TSP and ATL patients in the southwestern Japan, the Caribbean islands, the pacific coast of Colombia and the central Africa suggests a genetic background of the diseases segregating with a specific family or racial group. The genetic background of HAM/TSP patients was investigated by analysing their HLA haplotypes. The Japanese HAM patients had a typical Japanese HLA haplotypes which were commonly detectable in the general Japanese population. In contrast, HLA haplotypes of ATL patients were different from those of HAM patients. Namely, acute type ATL had an unique HLA haplotype which was very rarely detectable in the Japanese population. Thus, HLA haplotypes could discriminate the HTLV-I-associated disease entities, HAM and ATL. The similar genetic segregation was observed in TSP patients in the Caribbean and the Zairean population whose racial background is probably related to a special tribe of the central Africa.
