Development and clinical application of GHRP/GH secretagogue

　オピオイドペプチドの下垂体ホルモンの分泌機構の研究中に，その誘導体がGH分泌活性を有することが明らかにされ，これをもとにGHRP（GH releasing peptide）の開発が行われ，その後，非ペプチドのGH分泌促進物質（GH secretagogue：GHS）も開発された。その後，GHS受容体も解明され，内因性のリガンドであるグレリンが発見されるに至った。GHRP/GHSの強力なGH分泌促進活性を利用して，GH分泌刺激試験の診断薬として，また加齢に伴うGH分泌低下症の治療薬としての応用が考えられている。

はじめに

　GH分泌は視床下部よりのGHRH（GH releasing hormone），ソマトスタチンにより調節されるが，この2つの因子のみではGH分泌調節を説明しえない成績が少なからず存在し，GHRH以外のGH促進因子が存在することが推定されていた。オピオイドペプチドの下垂体ホルモンの分泌機構の研究中に，その誘導体がGH分泌活性を有することが明らかにされ，これをもとにGHRP（GH releasing peptide）の開発が行われ，その受容体〔GH secretagogue：（GHS）受容体〕が解明され，GHS受容体の内因性のリガンドであるグレリンが発見されるに至った。

　本稿ではGHRP，GHSの開発の経緯とその臨床応用について概説する。なお，グレリンについては前項で述べられているので，本稿では簡単に触れるにとどめる。

　In studies of the opioid control of GH secretion, several peptide analogues of met-enkephalin（GHRP-6, hexarelin, GHRP-2, etc）were found to be potent GH secretagogues（GHS）. Subsequently, a series of nonpeptidyl GHRP mimetics（MK-0751, MK-0677）were synthesized with greater oral bioavailability. The receptor of these GHRP/GHS（GHS-R）that is unrelated to the GHRH receptor was cloned, and then the natural ligand of the GHS-R（ghrelin）was discovered. The GHRP/GHS are more potent on a weight basis than GHRH itself, and synergize with co administered GHRH and are almost ineffective in the absence of GHRH. For these properties, the GHRP/GHS have been considered for clinical applications of diagnostic test and treatment of GH deficiency in aging.
