 Human growth hormone-releasing hormone(hGHRH), originally isolated from pancreatic tumor that caused acromegaly, is a hypothalamic peptide consisting of 44 amino acid residues that stimulates both synthesis and release of growth hormone. HGHRH in plasma has been measured by radioimmunoassay after extraction with Sep-Pak cartridges or by immunoaffinity. The extraction methods are not only time-consuming but also cause great variation in assay results. We have developed a novel and ultrasensitive immune complex(antibody-antigen-antibody complex)transfer-enzyme immunoassay(ICT-EIA)that enables us to measure concentrations of human GHRH in plasma directly. The detection limits of the ICT-EIA is 400 zeptomole(zmol=1×10-21mol)/tube or 0.2pg/ml, which is 1,000 times more sensitive than those of conventional RIAs and far below the normal range of plasma GHRH levels in normal human subjects(4-14pg/ml). The ICT-EIA for hGHRH provides us an opportunity to elucidate the autocrine, paracrine and endocrine mechanisms by which GHRH develops GH-producing pituitary adenoma in hGHRH-transgenic rats in a time-dependent manner. When we screened a series of 300 patients with acromegaly with the hGHRH ICT-EIA, we found one patient with a GHRH-producing carcinoid tumor of the pancreas(incidence of acromegaly due to ectopic GHRH source is 0.3%). Using the ICT-EIA, we also found several unique natures of acromegalic patients with ectopic GHRH-producing tumors.
