 In this article, recent advance of genetic study of ischemic stroke have been reviewed.

 A certain number of ischemic stroke are single-gene disorders. Cerebral autosomal dominant arteriopathy with subcortical infarcts and leukoencephalopathy(CADASIL)is an autosomal dominantly inherited non-amyloid angiopathy caused by mutations in Notch3. Cerebral autosomal recessive arteriopathy with subcortical infarcts and leukoencephalopathy(CARASIL)is a syndrome of cerebral small vessel disease in combination with orthopedic problems and alopecia. Hereditary endotheliopathy with retinopathy, nephropathy, and stroke(HERNS)is an autosomal dominantly inherited microangiopathy of the brain in combination with a vascular retinopathy. One of the responsible gene has been mapped to 3p21.3-p21.1. Mitochondrial myopathy, encephalopathy, lactic acidosis, and stroke-like episodes(MELAS)is associated with several mutations in the mitochondrial DNA.

 Common forms of ischemic stroke seem to be caused from multifactorial genetic factors and environmental factors. The role of large number of candidate genes has been investigated in stroke using the method of case-control association study. The candidate genes can be conveniently grouped into five categories:1)the renin-angiotensin system, 2)nitric oxide production, 3)lipid metabolism, 4)hemostasis and prothrombotic state, 5)homocysteine metabolism.

 Recently, the genome-wide association studies from Iceland have been found the strongest association in the gene encoding phosphdiesterase 4D(PDE4D), especially for carotid and cardiogenic stroke.

 Discovery of new responsible or susceptible genes to ischemic stroke will also open an avenue to develop new therapeutic strategy of stroke.
