Multiple sclerosis(MS)is a chronic inflammatory demyelinating disease and autoimmunity to central nervous system myelin has been implicated in those patients. A number of studies have elucidated the immunological abnormalities associated with MS, which have enabled clinical trials using immunomodulatory drugs. However, no immunological tests have been utilized to assess the effectiveness of those treatments. If an immunological marker that changes in parallel with MS disease activity could be established, it would assist in deciding which treatment to choose and may show the central part of the immunological etiology of MS. Real-time assessment of cell-mediated immunity is easily performed using flow cytometry. Further, since tests with blood samples can be repeated frequently, they are useful for immunomonitoring on an outpatient basis. On the other hand, tests with cerebrospinal fluid samples provide important information, e. g., that associated with individual patient prognosis, even if performed annually. Among the various immunological markers tested and reported, those that indicate Th1/Th2 balance are thought to be very useful. Therefore, it is necessary to establish tests that assess the functions of CD8 cells from the viewpoint of suppressor cells as well as Tc1/Tc2 balance. In addition, humoral immunity has recently been revisited as a cause of MS subtype, and might soon be able to differentiate optic-spinal MS from classic MS. Thus, it is important to develop routine tests for detecting NMO-IgG and anti-myelin antibodies, while studies of dendritic cells should clarify the difference in pathogenesis between relapsing-remitting MS and primary progressive MS.
