同心円硬化症は大脳の急性脱髄疾患で，脱髄が同心円層状に起こる特徴がある。筆者はフィリピンにおいて本症の剖検例17例，臨床例30例を収集した。その臨床的特徴は無言（reticence），行動異常，四肢の脱力，錐体路徴候などである。髄液は正常ないし軽度の炎症所見を示し，MRIで広汎多巣性大脳白質病変を認める。脱髄病巣は壊死傾向が強く，炎症は軽く，辺縁のactive zoneに活性化マクロファージが多数みられる。周辺の髄鞘が保たれる部位に，低酸素により誘導されるHIF-1α，HSP70，D-110などの発現がみられ，低酸素/虚血型脱髄に分類される。本症は1980年代から1990年代前半にかけてフィリピンで多発したが，1990年代後半から激減し，最近ではほとんどみられなくなった。

Concentric sclerosis（Baló）is an acute demyelinating disease of the central nervous system, which is characterized by concentric alternation of demyelination and myelin-preserved zones. I have collected 17 autopsy cases and 30 clinical cases in the Philippines. Clinical manifestations are characterized by behavioral disorders, reticence, limb paralysis and pyramidal signs. Cerebrospinal fluid change is subtle and MRI shows multifocal large white matter lesions. Pathologically, the lesion is necrotic with concentric preservation of myelin and axon, and lymphocytes infiltration is subtle. There is an active zone in the periphery of lesions, where numerous activated macrophages are present. In the surrounding area of lesions, hypoxia-induced factor-1α, HSP70 and D-110 are expressed. Thus, this is classified as the hypoxia/ischemia type demyelinating disease. It was prevailing in the Philippines, but abruptly disappeared recently.
