難治性てんかんに対する脳深部電気刺激療法

Deep brain stimulation for intractable epilepsy

多くの動物実験や過去のてんかんに対する定位脳手術によって得られたデータから，てんかんの焦点とは直接関係のない脳内構造の電気刺激がてんかん発作を抑制することが明らかとなり，脳深部電気刺激療法が新しいてんかんの外科治療として注目されている。電気刺激のターゲットとして注目されているのは視床下核と視床前核であり，すでに臨床研究も開始されている。しかし，その発作抑制機序についてはまだ不明な点も多く，基礎研究も並行して継続されている。この新しいてんかん外科治療法の基礎となる研究を筆者らの研究結果を含めてレビューするとともに，現在の欧米での臨床応用の結果についても報告する。

Lots of basic animal studies and results of the past stereotactic neurosurgery for epilepsy revealed that electrical stimulation of some brain structures, which are not related to epileptic focus, suppresses epileptic seizures. The deep brain stimulation（DBS）has attracted as a novel surgical treatment for epilepsy, however, the mechanism of antiepileptic effect of DBS has still unclear. We reviewed previous studies as background of DBS for epilepsy, showed some data from our experimental study, and present recent preliminary clinical results.
