周産期における向精神薬の使い方—胎児，乳児へのリスクとベネフィット

How to Use Psychotropic Drugs During the Perinatal Period: Risks and Benefits for Fetuses and Infants

周産期は精神疾患の発症・増悪のリスクが高まるが，胎児や乳児への影響が不安視され向精神薬による適切な治療が行われない場合がある。本論では，周産期の発症・増悪リスクが高い精神疾患を概説し，それぞれの疾患の標準的な薬物療法における胎児や乳児へのリスクとベネフィットについて解説した。また，薬物治療の際には，妊娠を見据えて患者・家族とともに事前に相談しておくことや，正しい情報を基にした共同での意思決定が重要であることを述べた。

Abstract

The risk of developing or exacerbating psychiatric disorders increases in the perinatal period. However, there is a concern that psychotropic medications may be withheld from appropriate treatment by doctors, or patients or their families because of concerns about their effects on the fetus or infant. This article describes psychiatric disorders with risk of perinatal onset or exacerbation and presents the risks and benefits of standard pharmacotherapy to the fetus and infant. It is essential to consult with the patient and family before conception to make shared decisions based on correct information.
