自然破裂をきたした腎細胞癌の2例

Spontaneous Rupture of Renal Cell Carcinoma:A Report of Two Cases

　腎細胞癌自然破裂の2手術例を報告する。症例1は健康な51歳男性に急性腹症で発見された腎被膜外破裂で，症例2は11年前に指摘された偶発癌経過観察中の70歳男性に血尿，側腹部激痛をもって発症した被膜内破裂である。腎細胞癌自然破裂の術前診断は困難で，本邦報告31例中19例のみで正診であった。症例1では破裂後の，症例2では破裂前後のCTで診断した。腎自然破裂の基礎疾患は腎細胞癌が最も多いので治療は根治的腎摘除術が望ましい。

We report two surgically treated cases of renal cell carci-noma with nontraumatic rupture. The first patient is a51-year-old man,who was hospitalized because of acute abdomen. Enhanced abdominal CT presented a right renal tumor and extracapsular hematoma. The sec-ond case is a 70-year-old man who complained of rightflank pain and gross hematuria. Six months previously right renal cancer had been diagnosed by CT and angi-ography. The CT after the symptom onset demon-strated subcapsular bleeding which deformed the renal parenchym.
