回腸利用膀胱拡大術を施行した間質性膀胱炎

A CASE OF INTERSTITIAL CYSTITIS TREATED WITH ILEAL BLADDER AUGMENTATION

　外科的治療で満足のいく結果を得た間質性膀胱炎を経験した。症例は53歳，女性。頻尿，膀胱充満時下腹部痛，排尿時痛を主訴に1985年7月，当科受診。間質性膀胱炎としてステロイド剤などの薬物療法や水圧療法を行ったがほとんど効果なく，1992年9月，膀胱三角部を残した膀胱部分切除術および回腸を利用した膀胱拡大術を施行した。術後7か月で最大尿意容量300mlとなった。術後36か月現在，排尿間隔は2時間程度で，排尿痛，排尿困難や尿失禁はなく職場に復帰している。

A 53-year-old woman with interstitial cystitis was suc-cessfully treated with supratrigonal partial cystectomy and ileal bladder augmentation. She was unresponsive to conservative treatment. Preoperative maximal desire to void was 60 ml, which increased to 300 ml seven months after operation. The patient became completely free of all symptoms in 3 years after operation.
