右副腎巨大褐色細胞腫

Giant adrenal pheochromocytoma with hyperglicemia:A case report

　患者は49歳，女性。約20年前から高血圧を，約2年前から耐糖能異常を指摘されていた。CT,MRlにて右副腎部に巨大腫瘍を認め，血中ノルアドレナリンが異常高値を呈した。MIBGシンチグラフィーで同部に異常集積像を認め褐色細胞腫と診断し，右副腎摘除術を施行した。腫瘍重量1,370g。術後高血圧および耐糖能異常は速やかに改善した。

A-49-year-old woman was adrnitted to our hospital with palpitation-Both CT and MRI revealed right adre-nal huge tumor. Her serum and urinary norepinephil-rine were highly elevated. In addition, hyperglicemia was also found. Right adrenalectomy was performed through transabdominal approach with subcostal inci-sion. The weight of the resected tumor was 1370 g and histpathological finding showed pheochromocytoma. Hyperglicemia was quickly improved after the operation.
