両側尿管結石症例における治療法の検討

Significance of ESWL for the Treatment of Bilateral Ureteral Stones

　両側尿管結石に対するESWLを中心とした治療方針を確立するため，1994年6月から1999年5月までに経験した20症例47結石を対象に検討を行った。初診時に腎機能障害例を25％に認めた。結石完全排石率は80％で，治療中重篤な合併症は認めなかった。砕石困難の原因として結石サイズ，水腎症の程度が関与しており，ESWLの治療のみで砕石困難な場合，TULによる内視鏡手術への変更が必要であった。一方でin-situ ESWL治療の有効性が示された。適応は両側水腎症が中等度以下，結石サイズが10mm前後でIVUで結石以下の尿管描出が見られれば片側尿管結石と同様に考え，ESWL単独治療を試みて支障なしと考えられた。

In order to investigate the efficacy of extracorporeal shockwave lithotripsy (ESWL) for the treatment of bilateralureteral stones, we analyzed retrospectively the resultsof this procedure in 20 patients with 47 stones. The overall stone free rate was 80% and no severe complications were noted. Renal dysfunction was seen in 25% of the patients at the initial examination. The success rate was dependent upon the stone size and degree of hydronephrosis, and sulbsequent TUL was necessary when in-situ ESWL alone was unsuccessful.
