内視鏡的治療を施行した小児シスチン膀胱結石

The Endoscopic Management for Infantile Cystine Calculi of Bladder:Report of a Case

　症例は1歳11か月の男児。主訴は排尿時痛。KUBにて25×15mmの膀胱結石を認め精査・加療目的で当科入院となる。ESWL施行するも砕石は不完全であった。後日内鏡視下にEHL，尿管鏡用生検鉗子を使用し結石を抽出した。結石分析の結果はシスチン結石であった。小児膀胱結石の内視鏡的治療は安全かつ有効であり，尿管鏡用生検鉗子が補助的手段として有用であることが示唆された。

We herein report the case of 1-year-11-month old boywith cystine calculi of bladder. Radiological examination(KUB) showed a large bladder stone (25×15mm). Atfirst,we perfomed extracorporeal shock wave lithotripsy(ESWL) under general anesthesia, but was not successful. Therefore, electrohydraulic lithotripsy (EHL) was done,secondly using biopsy forceps for ureteroscope to removesmall stones. Open vesicolithotomy has been performedin almost all of infantile bladder calculi.in Japanese articles.Endourological procedure has rarely been reported.
