臨床病期Ⅰの非セミノーマ精巣腫瘍に対する神経温存後腹膜リンパ節郭清の成績

The Results of the Nerve-sparing Retroperitoneal Lymphadenectomy to Clinical Stage I Nonseminomatous Testicular Tumors

　臨床病期Iの非セミノーマ精巣腫瘍患者7例に対して神経温存後腹膜リンパ節郭清術を行い，術後の射精機能および再発の有無を検討した。患者の平均年齢は30.7歳で，術後の観察期間は平均56.3か月であった。7例中6例で術後の順行性射精が観察され，2例で3人の挙児が得られた。術後1例に肺転移が出現し，化学療法後に肺切除が行われた。後腹膜再発は皆無で，全例癌なし生存であった。本術式は，治療効果と射精機能温存の面から有用である。

The nerve-sparing retroperitoneal lymphadenectomy was done in 7 patients with clinical stage I nonseminomatous testicular tumors. We evaluated its postoperative ejacula-tory function and possibility of tumor recurrence. The patients were 22 to 43 years old (mean age 30.7). The follow-up period was 21 to 90 months (mean 56.3). In 6 patients, antegrade ejaculations were observed and 3 babies were born in 2 patients after the lymphadenectomy. In one patient, lung metastasis was detected after the lym-phadenectomy, and he underwent chemotharapy and tumor resection.
