無症候性顕微鏡的血尿に対する2次スクリーニングの意義

Clinical Significance of Secondary Screening for Asymptomatic Microscopic Hematuria

　尿潜血反応陽性のために当科を受診した750例に対して，2次スクリーニングとして尿検査，血液検査および超音波断層検査を中心に行い，尿路性器悪性腫瘍3例，治療を必要とした尿路結石症19例，前立腺肥大症6例，尿路感染症4例，海綿腎2例，嚢胞腎1例，膀胱尿管逆流症1例，腎の血管筋脂肪腫1例，間質性膀胱炎1例，尿道小丘1例，血清IgAの高値65例が発見され，これらは受診者の13.9％を占めた。健康診断によって発見される尿潜血反応陽性者または顕微鏡的血尿を有するものについて，侵襲の少ない検査を選択して積極的に精査を行うことは，尿路性器疾患の早期発見のために意義あることと考えられた。

Asymptomatic microscopic hematuria is a relatively com-mon clinical condition that requires urologic evaluation. To evaluate the etiology of microscopic hematuria, the cases of 750 consecutive patients with asymptomatic microscopic hematuria who had undergone urinalysis, urinary cytology, measurements of serum immunoglobulin and ultrasonography, were reviewed. Their ages ranged from eighteen to seventy-eight years (average 45.9 years). Significant urologic lesions were found in 13.9% of patients. Three of these patients had a genitourinary malignant neoplasm.
