第三世代の電磁変換方式結石破砕装置による尿路結石の治療経験

Treatment of Urinary Calculus by Modulith SLX

　筆者らは，大和病院泌尿器科でModulith SLXの臨床経験をした。症例は男性30例，女性19例の49例で，年齢は17〜84歳（平均45.9歳）であった。結石は腎結石が18例，尿管結石が31例，大きさは4mm以下が3例，5〜10mmが28例，10mm以上が18例であった。原則として外来通院とし，ジクロフェナクナトリウム50mg坐薬挿肛以外は一般的に無麻酔で行ったが，1例に硬膜外麻酔，2例に腰椎麻酔を施行した。治療効果は術後1か月のKUBにて判定し，有効率は85、7％であった。主な副作用は疹痛，血尿などで，特に治療を必要としなかった。Modulith SLXは無麻酔にて結石破砕を短時問にでき，有用な治療方法であると思われた。

We have experienced ESWL using Modulith SLX at the urology department of Yamato Hospital, in 49 patients,30 male and 19 female. The age of the patients ranged from 17 to 84 years old. The culculi consisted of 18 renal calculi and 31 ureteral calculi. The size of culculi was 4mm or less in 5 case,5 to 10mm in 26 case,10mm or more in 18 case. The patients were mainly treated on outpatient basis. The treatment was conducted without anesthesia as a rule, and analgesic suppositories were used preoperatively. However, epidural anesthesia to one case and lumber anesthesia to two cases were needed.
