二重尿管カテーテルを用いた尿管結石に対する体外衝撃波砕石術

　筆者らは，体外衝撃波砕石術（ESWL）時に破砕を促進させるため，鞘としての外カテーテルと内側を自由に動き，常に先端部を結石に密着させ，注水による水圧効果を最大限に生かした内カテーテルの二重カテーテルを考案し，使用した。

　対象は，1991年3月〜1992年8月までに行われたESWL治療症例のうち，術前X線検査および現病歴により，破砕しにくいと判断した尿管結石男性41列，女性17例の58症例である。その結果，1回のESWL単独療法による破砕有効例が58例中54例（93％）と高い有効率であった。以上の成績から，二重尿管カテーテルを併用することはESWL単独療法に際し，尿管結石に対する有効な治療法であると考えられる。

We devised a double ureteral catheter to facilitate fragmentation of ureteral stone during extracorporealshock wave lithotripsy (ESWL). The inner catheter can move freely within the outer catheter whichfunctions as a sheath. The tip of inner catheter is manuplated to touch the stone at all times, in order tomaximize the hydraulic effects of the injected water through the catheter. With the use of this catheter 58patients (41 male and 17 female patients) were treated who were diagnosed as having ureteral stones difficultto crush with a single ESWL treatment.
