経尿道的前立腺切除術後のDダイマー測定の意義

STUDY OF D-DIMER FOLLOWING TRASURETHRAL RESECTION IN PATIENTS WITH BENIGN PROSTATIC HYPERPLASIA

　経尿道的前立腺切除術による二次線溶の変動を調べるため，前立腺肥大症15例において術前術後に血清のDダイマーを測定した。術後1日目の血清Dダイマーは，術前に比べ統計学的に有意に上昇していた。術後書1間目では，Dダイマーは2例を除いて減少傾向にあった。他の凝固系検査では，消費性の凝固障害を示す所見は認められず，血小板の急激な低下も認められなかった。以前より前立腺の手術により線溶系が亢進すると考えられていたが，今回の結果は前立腺肥大症に対する経尿道的切除術により，凝固系の活性に引き続いて起こる線溶系の活性（二次線溶）が生じていることを示していた。

To study changes in secondary fibrinolysis due to trasurethral prostatectomy, serum D-dimer levels were measured before and after surgery in 15 cases of benign prostatic hypertrophy. Serum D-dimer levels of all 15 cases measured the day after surgery exceeded the standard levels of 150 ng/ml, showing a statistically significant increase compared to the preoperative levels. All but 2 cases showed a tendency for serum D -dimer levels to decrease one week after surgery.
