前立腺生検時における膀胱鏡検査で発見された膀胱腫瘍の検討

Study on coincidental bladder tumor found by cystoscopy at prostate needle biopsy

　2003年1月～2008年3月までに当院で前立腺針生検を施行した791例において，同時に施行された膀胱鏡により偶然発見された膀胱癌についての検討を行った。23例が膀胱腫瘍と診断され，同時に経尿道的膀胱腫瘍切除術が施行された。その病理学的診断は膀胱癌が15例，前立腺癌が1例，良性上皮性腫瘍が1例，良性炎症性病変が6例であった。施行された生検により確定された前立腺癌は322例で，膀胱癌との同時性重複癌は6例であり，合併率は1.9％（6例/322例）であった。偶然発見された膀胱癌はいずれも臨床病期は早期であり，内視鏡的に切除可能であったことから，前立腺針生検の際に膀胱鏡検査を行うことは臨床的に意義があることが示唆された。

　Cystoscopic examination is carried out simultaneously with prostate needle biopsy at our hospital. Toatal 791 patients,who had underwent prostate needle bipsy from January 2005 to March 2008,were analyzed retrospectively as to the coincidence with bladder cancer. The 23 bladder tumors,diagnosed clinically were resected transurethrally at the same time. They were diagnosed pathologically as 15 bladder cancers,1 prostatic cancer,1 benign epithelial tumor and 6 benign inflammatory tumors. Prostate cancer was confirmed in 322（40.7％）. The number of patients with both prostate and bladder cancers were 6 and the coincidence rate was 1.9％. All of the bladder cancers among the subjects showed low stage and was transurethrally resectable. It is,therefore,suggested that simultaneous cystoscopy with prostate biopsy may have a clinical benifit.
