遠隔皮膚転移を来した前立腺癌の1例

A CASE OF PROSTATIC CARCINOMA WITH METASTASIS TO THE SKIN

　前立腺癌の皮膚転移は極めて稀なものとされ，本邦ではIchikawaらの報告以来，5例をみるのみである。今回，われわれは，遠隔皮膚転移を初発症状とする前立腺癌の1例を経験したので報告する。症例は79歳，男性。皮膚腫瘤の生検より前立腺癌の皮膚転移が疑われた。前立腺生検により未分化型前立腺癌の診断を得，diethylstilbestrolの投与および除睾術を行い，臨床症状の著明な改善をみた。

A 79-year old man was admitted to the division of internal medicine at Otaru City Hospital with a complaint of nodular lesions of the skin, general weakness, neuralgic pain and anemia. There were cutaneous nodules involving the face, head, and breast. Examination by palpation revealed that the prostate had rock-hard nodules. Blood chemistry showed markedly elevated acid phosphatase. Results of a radioisotopic bone scan showed skeletal metastasis. A biopsy of a skin lesion revealed adenocarcinoma identical to that of the prostate. The diagnosis of carcinoma of the prostate with metastases to the skin and bone was established.
